If you prefer to fill out the patient information forms in our office, please arrive to your first appointment at least 15 minutes early. You will also need to bring your physician’s prescription and insurance information to your first visit, so that we can bill your insurance provider.
Occasionally we may not have the amount of staff to accommodate the number of referrals we receive. If this is the case, we will gather your information so we can contact you and begin services when a space opens up. We can also provide you with contact information for other rehabilitation services in the area if you need care right away. Unfortunately, our patient turnover, especially among our pediatric patients, is sometimes slow, we may not be able to give an estimate as to how long the wait time will be. But we can assure you that we are committed to providing you with the same attentive, personalized care.
We are happy to bill our patients’ insurance providers, and we accept all insurance forms, including Medicaid and Medicare. We are a preferred provider for Blue Cross/Blue Shield and Tricare.
